Quality Improvement and Patient Safety (QIPS) Fellowship
The QIPS Fellowship at CMH is a one-year non-ACGME accredited junior faculty position designed to provide our residency graduates with clinical, academic, and administrative training in quality improvement, patient safety, and leadership development. This collaboration between our clinical and Quality Services departments provides an opportunity for early career physicians to become involved in medical education and hospital administration and learn the skills to lead changes in health care.
The fellowship includes, but is not limited to:
Leadership role to facilitate alignment with institutional QIPS initiatives, lead patient safety rounds with residents, and prepare for key areas of institutional CLER visits
Scholarship, through designing and presenting/publishing one clearly defined mentored longitudinal project with the goal of systematic improvements at CMH
Time as clinician-educator as inpatient hospitalist and/or in clinic
Opportunity to attend a regional or national QIPS conference
Participate in key CMH hospital committees, RCAs, and resident QIPS teaching and project mentorship
Sam Jazayeri, DO
Internal Medicine
3rd Year
Dr. Sam Jazayeri was born in Tehran, Iran and immigrated to the United States at the age of 16. He received his undergraduate degree in Physiological Sciences from University of California Los Angeles. He then worked as a Biological Lab Tech at the Greater Los Angeles Veteran Affairs Hospital before starting medical school at Western University of Health Sciences in Pomona California. As a medical student, Dr. Jazayeri was fortunate to be selected as one of the Pre-Doctoral NMM/OMM Teaching Fellows where he spent one additional year at Western University to help teach the Osteopathic Manipulative Medicine course to first- and second-year medical students. Dr. Jazayeri completed his internal medicine residency at the Community Memorial Healthcare in Ventura. While at CMH, he has developed an interest in quality improvement research projects and is very happy to have the opportunity to serve as the Quality Improvement and Patient Safety fellow for this upcoming year. In his spare time, he enjoys relaxing with friends, watching films, and listening to music. Dr. Jazayeri is proud to join the team at Community Memorial Health and is excited to serve the Ventura community.
